VETERANS HOME CARE (VA Community Care Network)

As a VA CCN‑contracted home care provider, Living Well supports veterans with in‑home assistance authorized through the VA. When care isn’t available through VA facilities—or travel is difficult—CCN connects eligible Veterans to approved community providers for services such as homemaker and home health aide support and respite. We coordinate closely with VA case managers to align care plans, authorization hours, and schedules.

Support we provide:

Help with activities of daily living (bathing, dressing, mobility, toileting, meal prep), safety monitoring, companionship, and caregiver relief.

DEMENTIA AND ALZHEIMER'S DISEASE CARE (GUIDE Partnership)

Alzheimer’s disease or other types of dementia can strike dread in the hearts of family caregivers. If you’re living it now, you know all too well. If you’ve just received the diagnosis for your loved one, you may feel scared and overwhelmed.

You Are Not Alone.
There is support for you and your loved one. At Living Well Senior Care, we combine compassionate care with advanced training and resources to help families navigate this journey. We have partnered with Relias training and maintain Certified Dementia Care Practitioners on staff to ensure our caregivers deliver the highest quality of Alzheimer’s and dementia care. 

Our caregivers have a passion for working with Alzheimer’s and dementia clients and receive ongoing classroom and online training. They bring the following skills and support to families:

  • Remembering Life’s Journey – Gathering past stories and experiences helps caregivers provide comfort and personalized care while honoring who the senior was earlier in life.

  • Techniques to Manage Behavior – Using strategies like offering simple choices and redirecting, caregivers help seniors remain calm and safe.

  • Encouraging Interaction – Engaging seniors throughout the day builds self-esteem, enhances physical strength, and reduces challenging behaviors.

  • Supporting the Family – Our caregivers understand the struggles of caring for a loved one with dementia. They maintain open communication and work closely with families to ensure quality care.​

We continually attend classes on the latest innovations to improve dementia care.

Enhanced Dementia Care Through GUIDE
In addition to our specialized training, Living Well Senior Care has partnered with Arcy Supportive Care to deliver enhanced dementia care through Medicare’s GUIDE (Guiding an Improved Dementia Outcome) program. This program offers coordinated care, caregiver education, and annual respite benefits—up to $2,500 per year for qualified patients—helping families access much-needed breaks without added cost.
